Patient Portal My Chart

We are excited to be switching to EPIC medical health records.  Our patient portal will be unavailable from 9/6/17 until 12/10/17.  We apologize for any inconvenience.  Please contact our office if you need a copy of your immunization record or need to leave us a message to call you back.  We hope with EPIC implementation, you will have a much better user interface experience using the patient portal.

We apologize for any inconvenience this may cause and hope to have the new system in place as soon as possible in December 2017.
